The Community Hospice & Health Services Foundation is dedicated to raising funds and awareness in support of CHHS mission, to embrace individuals and families facing life-changing journeys, enhancing quality of life for all. The CHHS Foundation works to enhance the quality of life for individuals and families facing life-changing journeys by supporting the expansion of hospice, palliative care and grief support services. Through fundraising efforts such as special events, mailings, grant writing, and memorial giving, the Foundation helps ensure the continued delivery of compassionate care to our community.

Mission Possible – Now & Forever Campaign

In April of 2024, CHHS Foundation launched our Mission Possible, Now & Forever Major Donor Campaign to raise funds to better meet the growing needs of our community members. With Tom Eakin as our Campaign Chairman and a fantastic group of community leaders stepping forward to form the Leadership Cabinet, we were off to a terrific start. These individuals paved the way, igniting action and serving with passion for others to join them in making contributions in support of the following initiatives.

The Mission Possible campaign has four focus areas, three supporting programs that contribute to healthcare and well-being NOW, and a fourth that will provide ongoing support for the future – FOREVER.

NOW:

FOREVER:

FOCUS 1: Investing in our core services in order to provide the expected high-quality care for the growing number of Baby Boomers who will be needing end of life care through our hospice and palliative care programs. Data indicates this will be a huge impact on our services.

FOCUS 2: Expansion of mental health and grief support services in our community by developing a Hope Counseling & Bereavement Center. This includes a plan for clinical supervision to increase our mental health workforce in the Central Valley which needs more qualified providers.

FOCUS 3: Pediatric Program expansion and modernization of the Alexander Cohen Hospice House (ACHH). With over 30 families at this moment receiving support for their seriously ill children we have identified a need to develop a Family Suite at ACHH. Additionally, as ACHH is now 20 years old, there is a need for patient facing modernization of our oxygen supply closet, the patient specific HVAC system and more.
FOCUS 4: Building the Endowment fund to ensure a strong future for Community Hospice & Health Services (CHHS). We will be seeking commitments from our broad donor base of people who have come to a decision to include CHHS in their bequests and estate planning. A newly developed Legacy Fellowship is being established to recognize these special donors.

By September 30,  2024 we had gifts and pledges surpassing the five-million-dollar mark!

Pam Phillips Memorial Drive-Thru BBQ Fundraiser

The Pam Phillips Drive-Thru BBQ continues to be a cornerstone of community support for Camp Wokini, a youth grief camp for children and teens ages 6-17, offered by CHHS. Held annually, this event honors the memory of Pam Phillips, who, with her husband Kenny, started the BBQ to raise vital funds for children who have experienced the death of a close loved one.  

In 2024, 330 tickets for the BBQ were sold, a testament to the generosity and commitment of our community. In total, the event raised over $20,000 in sponsor donations and meals purchased. The BBQ event’s success is a direct reflection of the enduring legacy of Pam and Kenny’s vision, which has allowed us to continue offering Camp Wokini to children in need of support and healing.

Our next Drive-Thru BBQ will be held on April 25, 2025, from 4:00-6:00 pm at our Spyres Way location in Modesto. Please join us for some fantastic BBQ! We look forward to seeing you there!

Annual Sapphire Gala

The Annual Sapphire Gala held on May 18, 2024, marked a truly special milestone for CHHS, celebrating 45 years of compassionate care and community support. Hosted at the Mark Gallo Health & Fitness Center at Central Catholic, the event brought together 432 guests for an unforgettable evening of reflection, celebration, and fundraising. 

Guests were treated to a delicious dinner, while being immersed in the heart of our celebration: a living timeline showcased the incredible journey of CHHS over the past four decades. From our humble beginnings to becoming a trusted provider of compassionate hospice and palliative care for adults and children, the timeline highlighted the countless lives touched, the services expanded, and the growth that has been made possible through the unwavering support of our community. 

Thanks to the generosity of our guests, our Gala raised $191,084. These crucial funds directly support our hospice and palliative care services, ensuring that we may continue providing high-quality, compassionate care to those who need it most.

We are deeply grateful to everyone who attended, donated, volunteered, and supported our annual Gala. Together, we continue to make a meaningful impact, helping to provide comfort and dignity for individuals and families during their most vulnerable moments. We look forward to seeing you at next year’s Gala, An Evening in Versailles, to be held on May 10, 2025 at the Mark Gallo Health & Fitness Center at Central Catholic High School in Modesto.

Hops & Vines

This year’s Hops & Vines brought the community together in a unique and festive way, furthering our ability to deliver compassionate care to those in need. The third Annual Hops & Vines event was held on August 17, 2024, at CHHS Spyres Way campus. This event brought together guests for a fun and memorable evening in support of our mission. Guests enjoyed a diverse selection of locally brewed ales and local wines, complemented by delicious bites, all while participating in activities like ax throwing, music provided by The Hawk radio station, and capturing moments in our photo booth.

Thanks to the generosity of our guests, $33,156 will directly support the essential services provided by CHHS. We are deeply grateful to everyone who attended and helped make this event such a success!

Golf Fore Community Hospice & Health Services

The Golf Fore Community Hospice & Health Services tournament, held on September 16, 2024, at Del Rio Country Club, was an exciting day of golf, community spirit, and fundraising. Golfers kicked off the event with delicious breakfast burritos provided by Sierra Ocean, LLC, before hitting the course for a round of golf. The excitement culminated with the highlight of the day—a thrilling helicopter ball drop that added an extra dose of fun to the event. After the golf round, participants enjoyed a flavorful barbecue dinner, followed by a raffle and live auction, with proceeds totaling $49,156.70 going directly to support the mission of CHHS.

We are deeply grateful to everyone who participated in our Golf tournament and made this event so impactful. Save the date for next year’s tournament (as this event sells out quickly), which will be held on September 15, 2025—we look forward to another memorable and successful event! 

Walk & Roll Fun Run

On October 22, 2023, the CHHS Foundation, in partnership with Friends of CHHS, hosted the Walk & Roll Fun Run, a community event dedicated to supporting the pediatric hospice and palliative care programs offered by CHHS. Over 350 families, friends, and supporters joined us for a day of fun, fitness, and philanthropy, raising awareness and crucial funds for these essential services.

A special thank you to all who participated and contributed, helping us create a positive impact for local children and families in need. Our Friends Volunteer Auxiliary group earned $58,981 through both registration fees and sponsor donations. We are excited to introduce team participation at our next event!

Veterans Pavilion Celebration

On November 10, 2023, at 11:11 A.M., the CHHS Foundation gathered at the Veterans Pavilion at our Spyres Way campus for a heartfelt Veterans Day ceremony. This special event honors all veterans who have served our country. A new flag was raised in honor and memory of Jordan James Herr, a local hero whose service and sacrifice continue to inspire our community. The flag-raising in Jordan's honor was a poignant moment that reflected the deep gratitude we feel for all those who have fought for our freedom.

We are thankful to everyone who participated and contributed to making this event a memorable and meaningful tribute.

As we reflect on the sacrifices of our veterans, we remain committed to supporting and honoring them throughout the year. CHHS is a proud member of the We Honor Veterans Program, meeting the unique needs of veterans and their families with specialized patient care and grief support. Thank you to all who joined us, and we look forward to continuing our mission to recognize and serve those who have given so much for our country.

Angel Garden Dedication

On April 6, 2024, the CHHS Foundation hosted a heartfelt Angel Garden dedication ceremony under the Rose Pavilion at the Alexander Cohen Hospice House. This annual event honors the lives of the children we have had the privilege to care for, offering a moment of reflection, love, and remembrance. This year, we dedicated bricks to 11 children, each whose memory continues to inspire and touch the hearts of all who knew them.

As we gathered in the peaceful beauty of the Angel Garden, we were reminded of the strength, love, and resilience of the children and families we serve. We are deeply grateful for the opportunity to honor their memory and will continue to hold them in our hearts.


Friends of Community Hospice & Health Services

The Friends of CHHS is a group of extraordinary individuals who work tirelessly to raise funds and awareness for the essential services provided by CHHS. Through their dedication, passion, and commitment, this group of volunteers has become a driving force behind many of our organization’s initiatives and events. Their efforts help ensure that local families have access to the compassionate and quality care they need during some of life’s most challenging moments.

As true champions for our community, the Friends of CHHS go above and beyond to support our mission, creating a lasting impact on those we serve. We are deeply grateful for their continued generosity and for the vital role they play in helping us fulfill our promise of compassionate care and support for all. Our Friends raised and contributed $170,803.60 to support the care provided by CHHS. Additionally, Friends raised over $100,000 to support the Pediatric Room development at ACHH, as part of the Mission Possible campaign. 


Light Up A Life Holiday Tree

This year, the Friends of CHHS Light Up a Life tree lighting ceremonies expanded to include the Oakdale and Turlock communities, joining our long-standing tradition in Modesto. These special events, held in honor of loved ones who have passed but whose memories continue to brighten our lives, provide a moment of reflection, hope, and community support.

On December 1, 2023, Turlock Memorial Park & Funeral Home hosted a heartfelt ceremony with Nancy & Scott Atherton serving as the honorary tree lighters. The following day, the Oakdale ceremony took place at the Oak Park, where Mel Bradley and Mayor Cher Bairos had the honor of lighting the tree. The Modesto event, held at Memorial Medical Center on December 3, 2023, featured longtime supporter Jeff Burrow as the honorary tree lighter. Each of these ceremonies was organized by our dedicated Friends of CHHS Auxiliary group, whose unwavering support makes these gatherings possible. We are grateful to everyone who participated in these moving events, as we continue to honor and cherish the memories of those we have lost.

Pick Your Passion

The Pick Your Passion employee fundraising campaign is an annual initiative that allows CHHS staff to directly support the various patient needs and services that make a difference in the lives of those we serve. In 2024, 226 people participated in the campaign, pledging over $45,081 to support crucial programs like our internal food pantry, cremation fund, Angel Fund for medical equipment, Camp Wokini youth grief camp, and the Alexander Cohen Hospice House. This campaign highlights the incredible dedication of our staff and Leadership in ensuring that patients and families receive the compassionate care they deserve, while also strengthening our community of support.
